INSURANCE

Payment Options (accepted insurance)

My Care Health Center is a primary care physician office with locations in Mount Clemens, Center Line, Clinton Township, and Madison Heights. If you have a disability or chronic health condition, our medical offices accept Medicaid and other insurance programs.

We accept Medicaid and CHIP (children’s Medicaid – CHIP covers children whose families don’t qualify for Medicaid but can’t afford to buy health insurance).

There are many programs administered by the Michigan Medicaid program for people of all ages. The Michigan Department of Community Health has information on:

  • Children’s Special Health Care Services (CSHCS)

  • Under 21

  • Supplemental Security Income (SSI)

  • Special Disabled Children

  • Healthy Kids for Pregnant Women

  • Group 2 Pregnant Women

We also accept traditional and PPO insurances.

 

If you do not have health insurance and do not qualify for Medicaid or other similar plans, My Care Health Centers accept other forms of payment.

Other payment methods include cash, check, credit card, or payment plans (eligibility requirements apply). We also offer Sliding Fee Scale Discounts based on your family size and income.

My Care staff is available to help patients determine if they are eligible for a variety of health benefits coverage options. These options my include special grant-provided services or public-funded health care coverage such as Medicaid, MI Child or Healthy Kids. We can also help you with enrollment.

The Office Charge for your visit will be determined by your income and household size. Charges for other services like lab tests, x-rays and immunizations will be added to the discounted Office Visit Charge.
